Femte besøg på Riget - Fifth visit at Riget Hospital

Vi skal igen indtil Arthurs nye læge. Jesper er på arbejde, så jeg har taget Arthurs farmor, Anne-lise, med, da jeg håber at vi sammen kan forstå mere af hvad lægen siger. Måske kan Anne-lise også stille nogle af de spørgsmål jeg ikke får stillet.

Vi kommer ind til lægen og sætter os på hans kontor. jeg fortæller at anfaldene er tiltagende, men fortsat kortvarige. Jeg fortæller, at de primært kommer, når Arthur sover, men at vi ikke ved hvad der tricker dem... Der er jo helt stille og Arthur sover i sin egen seng.

Lægen fortæller, at dynens milde berøring måske kan være nok til, at starte et anfald hos Arthur.
Han taler om søvnmyoklonier og infantile stereotypier. Jeg synes vi spørger ind, men da vi går derfra, forstår jeg stadig ikke hvad han har sagt.

Lægen fortæller gang på gang, at der findes masser af spædbørn med anfaldsfænomener, der går væk af sig selv inden for et par måneder.
Derefter sætter han Arthurs medicindosis op til 3 mg x 2 dagligt.

Jeg skal ringe om en uge og fortælle hvordan det går. 


Fifth visit at Riget Hospital

Again we are going to Arthur’s new doctor. Jesper is at work, so I’ve taken Arthurs grandmother (Jesper’s mother) Anne-lise with me, in hopes that we might understand more of what the Doctor says, together. Anne-lise might also be able to ask the questions I am not asking.

We arrive at the Doctor’s, and sit in his office. I tell him that the attacks are progressive, but continually brief. I tell him that they primarily happen, when Arthur is asleep, but that we don’t know what tricks them... It is all quiet when Arthur sleeps in his own bed.

The Doctor tells us that the duvets mild touch might be enough to start an attack in Arthur.

He speaks of sleep-myoclonics and infantile stereotypy. I find that we ask about it, but when we leave, I still don’t understand what he has said.

The Doctor repeatedly tells us that there are a lot of infants with attack phenomenons, which disappear by themselves in a couple of months.

Hereafter he increases Arthur’s medicin dosis to 3 mg x 2 daily.

I must call again in a week and tell how it goes.

Anfald på trods af medicin - Attacks despite medication

Jesper og jeg har siden sidste besøg på Riget noteret alle anfald ned på klokkeslæt, Arthur har haft 3-6 anfald om dagen, primært i forbindelse med søvn. Anfaldene varer forsat kun få sekunder og vi bryder dem straks vi hører Arthurs hakkende gråd.

Jesper and I have since our last visit at Riget Hospital notated every attack on the clock, Arthur has had 3-6 attacks a day, primarily during sleep. The attacks continue to only last a few seconds, and we “break” them as soon as we can hear Arthur’s stuttering cry.

Fjerde besøg på Riget - Fourth visit at the hospital

Vi skal til tjek på Riget hos Arthurs nye læge. Jesper er på arbejde, så jeg tager alene derind. Jeg er nervøs, da Arthur efter medicinstart fortsat har anfald. De er stadig kortvarige (få sekunder) og jeg kan fortsat bryde dem ved at rejse Arthurs krop op i siddende position. 

Vi ankommer til afdelingen for "Sjældne Handicaps" - En afdeling jeg bestemt ikke har lyst til at tilbringe tid på - eller for den sags skyld synes mit barn skal være tilknyttet. I venteværelset leger en masse børn - Mange af dem ser meget specielle ud, nogle sidder i kørestol, nogle taler underligt. Vi venter længe - Arthur sover i barnevognen - Jeg spiller bordfodbold med en knap-så-normal dreng (til hans store fornøjelse)...

Efter en time kommer lægen haltende ud (sådan går han...) for at hente os.
På sit kontor undersøger han Arthur. Han fortæller, at Arthur opfører sig helt normalt. Han bliver vejet til 4,7 kilo. Han undersøger Arthurs hænder, der nu er slået ud.
Lægen siger Arthur har normal tonus og normale senreflekser og normale reaktioner. Han fortæller, at Arthur har en antydning af startlereaktion ved berøring eller lydpåvirkning. Han mener at Arthur måske har haft det han kalder "shivering attacks".Arthur skal fortsætte samme medicin dosis indtil den 24/2 hvor vi skal til tjek igen.

Jeg har utroligt svært ved at forstå hvad lægen siger, både fordi jeg ikke kender hans termer og fordi jeg ganske simpelt ikke kan høre hvad han siger. Jeg spørger om jeg kan få kopi af Arthurs journal, i håb om at kunne læse mig til noget...



Fourth visit at the hospital

We are going to a check-up at Riget* with Arthur’s new doctor. Jesper is at work, so I am going there by myself. I am nervous, because Arthur still has startle attacks after he startes on the medication. They are still very brief (a few seconds) and I can still “break” them by positioning Arthur’s body in upright sitting position.

We arrive at the “Rare Handicaps” unit - A unit I certainly don’t want to spend time at - or for that matter want my child to be connected to. Alot of children are playing in the waiting area - Many of them look very special, some are in wheelchairs, some speak peculiarly. We wait for a long time - Arthur sleeps in the carrycot - I play foosball with a not-so-normal boy (to his excitement)...

After waiting an hour the doctor comes out limpingly (that’s how he walks...) to bring us in. In his office he examines Arthur. He tells me that Arthur acts perfectly normal. He is weighed to 4.7 kilos. He examines Arthur’s hands, which are now outspread. The Doctor says that Arthur has a normal tonus and normal tendon reflexes and normal reactions. He tells me that Arthur has an indication of startle reaction when touched or sound influenced. He believes that Arthur might have had what he calls “shivering attacks”. Arthur is going to continue with the same dosis of medicin until the 24th of February, where we will have our next check-up.

I find it unbelievably hard to understand what the doctor is saying, both because I don’t know the terms he is using, but also because I simply can’t hear what he is saying. I ask if I can have a copy of Arthur’s medical journal, in hopes of being able to understand some of it...

Tredje besøg på Riget - Third visit to the hospital

Både Jesper og jeg møder op på Rigshospitalet. Vi bliver igen taget venligt imod i Børnemodtagelsen. De henviser os til en stue, hvor vi skal vente på lægen. Kort efter kommer en sygeplejerske ind. Hun fortæller os, at lægen er spastiker og spørger om vi er okay med det? Det er en underlig form for advarsel, da hans fysiske fremtræden jo ikke burde betyde noget. Alligevel forsikrer sygeplejersken os om, at han er virkelig dygtig.

Kort efter kommer lægen ind. Jeg fortæller, at Arthur siden han har fået medicin (3 dage), har haft 9 anfald. Jesper fortæller, at han har bemærket, at Arthurs anfald kan starte ved en pludselig lyd.

Lægen kan anfaldene for Startle Reaktioner, men vi forstår ikke rigtigt hvad han mener.
Han bemærker, at Arthurs hænder er meget knyttede og undersøger dem nøje. 

Arthur får også taget blodprøver og vi får en mærkelig tissepose med hjem, så vi dagen efter kan aflevere en urinprøve (lidt af et projekt med et blebarn)...

Efter samtalen bestiller lægen Frisium mikstur - Arthur skal have 2mg 2 gange dagligt. 

Vi henter miksturen senere på dagen, Arthur skal have den sprøjtet ind i kinden med en engangssprøjte  - Det ligner og smager af solbærsaft - Arthur synes det er et hit!

Third visit to the hospital

Both Jesper and I go to the hospital. We are, again, greeted kindly by the reception area for children. They direct us to a room where we have to wait for the doctor. Shortly after, a nurse enters the room. She tells us that the doctor is a spastic and asks if we are ok with that? What a strange warning, since his physical appearance should not matter. Nonetheless, the nurse assures us that he is really good.

Shortly after, the doctor enters. I tell him that Arthur has had 9 seizures since he started on his medication (3 days ago). Jesper tells him, that he has noticed that Arthur’s seizures can be set off by sudden sounds.

The doctor calls the seizures for “startled reactions” but we don’t really understand what he means by that. He notices that Arthur’s hands are very tightly clutched and examines them carefully.

They draw blood to test and we get a weird wee bag home with us, so we can deliver a urin sample the next day (which is really not easy with diapers and an infant)…

After our conversation the doctor orders Frisium mixture – Arthur’s dose is 2 mg twice daily.

We pick up the mixture later in the day, the medication is to be dispensed into Arthur’s mouth with a disposable syringe – it looks and tastes like blackcurrant juice – Arthur thinks it’s a hit!

Andet besøg på Riget - Second visit to the hospital

Jeg bliver ringet op tidligt om morgenen og får at vide, at jeg skal tage ind på Riget med det samme, da de har en ledig tid til video- og EEG overvågning. Arthur har siden i går haft 15 anfald.

Jeg ankommer igen til Børnemodtagelsen, hvor jeg bliver taget venligt imod. Jeg bliver henvist til en anden afdeling, som jeg forstår det, er det en afdeling, hvor de er specialiseret i epilepsi.

På afdelingen tager overlægen imod mig. Han er omsorgsfuld og fortæller omhyggeligt hvad der skal ske. Han fortæller, at han godt kan forstå hvis jeg er bange, da min søn jo er meget lille. Han følger mig ind på en stue, hvor en dame (kender ikke hendes stillingsbetegnelse) venter mig.

Damen vasker Arthurs hovede, mens jeg holder ham. Hun finder en badehættelignende hue med små plastikdutter i. I midten af hver dut, er der et hul, de ligner på en måde perler. Huen er alt for stor til Arthur og damen fortæller, at han er den yngste patient hun har haft. Hun henter en ny hue i depotet, men også denne hue er for stor. Med plastertape, får hun klistret huen fast om Arthurs hovede. I hver af de hvide dutter, fylder hun med en sprøjte, gele i. Derefter sætter hun en ledning i hver enkelt af dem. Jeg lægger forsigtigt Arthur i liften, han falder straks i søvn. Damen retter et videokamera til, så vi kan se Arthur på skærmen.

Få minutter efter, får Arthur et anfald, anfaldet begynder, idet damen rør ved Arthur. Kort efter får Arthur endnu et anfald. Begge anfald varer få sekunder og stopper idet jeg tager Arthur op for at trøste ham.

Det er ikke nødvendigt at optage mere, Arthur skal dog beholde huen på til næste undersøgelse, der foregår på en anden stue. På gangen møder jeg igen overlægen. Han har set optagelserne og forklarer lynhurtigt, at der ikke kunne ses forandringer i Arthurs hjerneaktivitet under anfald, hvilket der kan hos epileptikere. Han fortæller, at han har set tusindvis af forskellige former for epilepsi. "Jeg ved ikke hvad din søn fejler, men jeg kan garantere dig for, at han ikke har epilepsi". Som den første, har han lagt mærke til, at anfaldene begynder ved stimuli i form af lyd eller berøring.

En del roligere, sætter jeg mig med Arthur på den nye stue, hvor to damer venter os. Arthur er helt vågen og følger nysgerrigt med, mens han igen får ledninger klikket i huen og bliver tilkoblet et apparat EEG apparat. De slukker lyset og starter et stroboskoplys, men Arthur reagerer slet ikke. Hans EEG er igen helt normal.


Vi bliver sendt tilbage til Børnemodtagelsen for at vente på lægen fra i går.
Da lægen kommer, foreslår han, at Arthur får noget medicin. Han vejer Arthur til 4 kg og fortæller, at han skal have Clobazam (frisium) 2 ½ mg dagligt. Jeg har på ingen måde lyst til at give mit spædbarn medicin, men forstår, at det er nødvendigt.

Arthur skal ses til kontrol igen den 9. februar.

Second visit to the hospital

I get a call early in the morning and are told, that I should go into the hospital right away, as they have a free timeslot for video- and EEG monitoring. Arthur has had 15 seizures since yesterday.

I arrive, again, to the reception area for children, where I am being met by nice staff. I am directed to another department, and as I understand, it is a department where there are epilepsy specialist.
In the department the chief resident receives me. He is caring and explains in detail what is about to happen. He says that he can understand if I’m a scared, since my son is still very small. He walks me into a room where a woman (don’t know exactly what she is meant to be doing) is waiting.

The woman washed Arthur’s head while I hold him. She finds a sort of bathing cap with small plastic dots in. In the middle of every dot there is a small hole which looks a bit like a pearl. The cap is far too big for Arthur and the woman tells me that he is the youngest patient she has ever had. She gets a new cap in the cupboard, but this one is also too big. With the help of sticky tape used for bandages she is able to make the cap stick to Arthur’s head. In every white plastic dot she adds a gel from a syringe. Then she adds a wire to every one of the little dots. I put Arthur down very carefully into the cot and he falls asleep immediately. The woman corrects the position of a video camera so we can watch Arthur on the screens.

Only a few minutes later, Arthur has another seizure, which is set off as the woman touches Arthur. Shortly after that, Arthur has another seizure. Both seizures lasts a few seconds and stops as I pick up Arthur to comfort him.

It is no longer necessary to record any more, but Arthur has to keep the cap on until the next examination which takes place in another room. In the hallway I meet the chief resident. He has seen the recordings and explains very briefly that there weren’t any changes to Arthur’s brain activity during the seizure, which is usually the case of patients with epilepsy. He says that he has seen thousands of different types of epilepsy. “I don’t know what is the matter with your son but I can guarentee you that he does not have epilepsy”. He is first one to notice that the seizures are set off by stimuli in the shape of sound or touch.

Feeling somewhat more calm, I sit down with Arthur in the new room, where two women are waiting for us. Arthur is wide awake and curiously watches everything as wires are attached to the cap again and connected to an EEG monitor. They turn off the light and start a stroboscope light, but Arthur doesn’t react at all. His EEG is completely normal.

We get sent back to the reception area for children to wait for the doctor from yesterday. When the doctor arrives he suggests that Arthur gets some medication. He weighs Arthur in at 4 kg and tells me, that he will be given Clobazam (frisium) 2.5 mg daily. I do not feel like giving my baby any sort of medication at all, but understands that it is necessary.

Arthur has a scheduled control again on February 9th.

Første besøg på Riget - First visit at Riget (Hospital)

Jeg kører barnevognen ind til Børnemodtagelsen på Riget. En sygeplejerske tager imod mig. Jeg forklarer hvorfor jeg er der. Hun kigger alvorligt på mig "Det er ikke meningen, at en lille baby på tre uger skal have sådan nogle kramper" siger hun. Jeg føler mig både lettet og forstået. Jeg viser hende de videoklip jeg har optaget og hun beder mig vente på lægen i venteværelset.

Jeg venter længe, drikker kaffe, saftevand, kaffe, saftevand. Sidst på dagen kommer en læge. 
Lægen noterer min beskrivelse af Arthurs anfald ned og laver en børneundersøgelse. Arthur får et anfald på 5 sekunder, som lægen heldigvis ser.

Jeg får at vide, at Arthur skal indlægges til video- og EEG overvågning, men at jeg kan tage hjem med Arthur til de har en ledig tid. Jeg får telefonnummeret til Børnemodtagelsen og går lettet derfra.

February 6th
First Visit at Riget* Hospital
I walk the carrycot into the Child Reception area at Riget. A nurse receives me. I explain what brought me there. She looks serious. “A three-week-old baby is not supposed to have these kinds of cramps” she says. I feel relieved and understood. I show her the video clips I have recorded and she asks me to wait for the doctor in the waiting area.
I wait for a long time and drink coffee, juice, coffee, juice. Late in the day, a doctor arrives. The doctor makes a note of my description of Arthur’s seizures and performs a child examination. Arthur has a 5-seconds seizure, which the doctor fortunately observes.
I am informed that Arthur will be admitted for video- and EEG- supervision, but for now I can take him home till they have an opening. I am given the telephone number for the Child Reception and leave relieved.

Andet lægebesøg - The second doctor visit

Jeg kommer til tiden. Er klar i venteværelset, da lægen kalder mig ind. Igen er hendes attitude ubehagelig. Jeg sætter mig hurtigt ned og rækker hende kameraet. Hun ser klippene igennem en enkelt gang. Så løfter hun telefonrøret og ringer til Rigshospitalet.
Da hun har lagt røret på, fortæller hun, at jeg skal tage ind på Rigshospitalet og at de venter Arthur i børnemodtagelsen.

Da jeg står på gaden ringer jeg til Jesper. Jeg har ingen følelser, fortæller bare, at jeg nu tager på Riget med Arthur. Jesper bliver enormt bange og spørger om han skal komme hjem fra arbejde. "Nej nej, det er fint, jeg klarer det, jeg ringer senere..." 

February 5th
The Second Doctor Visit
I arrive on time. Sit ready in the waiting area when the doctor calls our names. Again, her attitude is unpleasant. I quickly sit down and hand her the camera. She watches the video clips through once. Then she picks up the phone and calls Rigshospitalet.*
When she has put down the receiver, she tells me to go to the Rigshospital and that they expect Arthur in the Child Reception.
In the street outside the hospital I call Jesper. I have no emotions; just tell him that I will bring Arthur to Riget. Jesper becomes frightened and asks if he should take off work and come home. “No no, it’s fine, I’ll handle it, I call you later..”

Videooptagelse - Video Recordings

Jeg finder mit kamera frem og gør det klar til at optage videoklip. Anfaldene kommer kun når Arthur sover. Jeg ammer ham og han falder som forventet i søvn. Jeg lægger ham blidt ned i vuggen og venter.
Arthur har ikke sovet ret længe, da første rystelse kommer. Det er et kort anfald og jeg når ikke at filme det. Arthur får flere anfald, men de varer kun få sekunder. Jeg kan ikke finde ud af hvad der tricker dem, men det må være noget der foregår i søvnen. 
Først sent om aftenen lykkes det mig, at filme tre anfald. Optagelserne er skræmmende og jeg bliver frygtelig ked af at se dem.
Hele natten, har jeg kameraet fremme, så jeg kan filme anfaldene. Jeg vågner hver gang af lyden af Arthurs hakkende gråd, men når kun at filme det sidste af anfaldene før de giver slip i hans krop.

February 4th
Video Recordings
I find my camera and prepare it to recording. The seizures only occur when Arthur sleeps. I feed him and he falls asleep as expected. I lay him gently in the cradle and wait.
Arthur has not slept for long when the first seizure occurs. It is a short seizure and I do not manage to film it. Arthur has several seizures but they only last a few seconds. I cannot figure out what the tricks them, but it has to be something that happens in his sleep.
Not until late at night, I succeed in recording three seizures. The recordings are frightening and I become terribly sad by watching them.
All night I have the camera prepared to film the seizures. I wake up every time by the sound of Arthur’s stuttering cry, but only manage to record the last of the seizures before they relief his body.

Første lægebesøg - The first doctor visit

Jeg kommer for sent til Arthurs lægeaftale. Amning og overtøj er gået i kludder. Jeg er stadig så nybagt-mor, at tid er et fremmedelement i min kalender. Jeg haster ind i venteværelset og undskylder forgæves til lægesekretæren. 
Lægen kommer ud i venteværelset og vrisser ad mig, at jeg skal komme til tiden. Hun er i hurtig bevægelse og synes travl. Jeg får bakset liften med Arthur ind i konsultationsrummet og sætter mig overfor lægen. Hun kigger stift på mig, som om jeg næsten forstyrrer hende ved at komme... Jeg får en fornemmelse af, at hun synes jeg er helt åndssvag og burde være blevet hjemme med Arthur. Alligevel får jeg forklaret, at jeg synes Arthur ryster på en underlig måde. Hun kigger mærkeligt på mig, ser ind i sin computerskærm og igen tilbage på mig. "Ryster?" - spørger hun... 
Jeg fortæller, at sundhedsplejersken har opfordret mig til at gå til lægen med Arthur, nærmest for at undskylde, at jeg overhovedet er mødt op. 

"Der er rigtig mange børn der har ubetydelige anfald, de går over"  - fortæller hun mig kortfattet. "Han har jo ikke rystet mens jeg har set på ham!" - fortsætter hun. Jeg bliver insisterende og hun bliver yderligere irriteret og nævner, at jeg jo er kommet for sent og hun skal videre. "Du kan komme igen, når du har optaget en video af det du synes der er galt" - afslutter hun. Jeg løfter liften med Arthur og skynder mig ud i venteværelset, løber ned ad trapperne, får sat liften i barnevognen og skynder mig ud i januarkulden.

February 3rd
The First Doctor Visit

I am late for Arthur’s doctor’s appointment. Breast feeding and warm coats are a mess. I am still so new at mothering, that time is alienated from my calendar. I rush into the waiting area and apologise to the doctor’s secretary without luck.
The doctor enters the waiting area and snaps at me that I ought to be on time. She is in fast movement and seems busy. I manoeuvre the carrycot with Arthur into the consultation room and place myself opposite, facing the doctor. She looks at me stiffly, almost as if I interrupt her by coming. I get the feeling that she finds me idiotic and thinks I should have stayed at home with Arthur... Nonetheless I manage to explain to her, that I think Arthur shakes in a strange way. She gives me a peculiar look, looks into her computer screen and then back at me, “Shakes?” she asks...
I explain that the Healt Visitor told me to take Arthur to the doctor, as a sort of excuse for my presence in her consultation room.

“Many children have insignificant seizures, it will pass in time” - she tells me briefly. “After all, he has not been shaking during our consultation!”, she continues. I become insisting and she becomes further annoyed and mentions that it was I who came late and that she had to move on. “You can come back when you have recorded a video of what you think is wrong”, she finishes. I lift the carrycot with Arthur and hurry myself out in the waiting area, run down the stairs, place the carrycot in the pram and rush myself out in the January cold.

Sundhedsplejerskens andet besøg - The Health Visitors’ Second Visit

Arthur er 3 uger gammel. Det seneste døgn, har han haft 14 anfald. Hans rystelser bliver voldsommere og voldsommere. Han nærmest kramper. Det sidder i hele hans krop. Jeg er dødsens angst for hvad der sker med ham. 

Sundhedsplejersken kommer til tiden. Jeg har igen lavet the, men Hanne Dorthe tager ikke noget. Jeg forklarer hurtigt, at rystelserne er blevet værre og hyppigere. Hun kan se på, at jeg er bange og opfordrer mig til at kontakte lægen.

Jeg bestiller tid hos lægen sekundet efter hun er gået. 

February 2nd
The Health Visitors’ Second Visit

Arthur is three weeks old. He has had 14 seizures in the last 24 hours. His shakings are increasingly severe. It is almost like he is convulsing. It is in his entire body. I am frightened to death of what is happening to him.

The Health Visitor arrives on time. I have made tea, again, but Hanne Dorthe does not seem to want any tea. I quickly explain that the shakings have become worse and more frequent. She can see that I am scared and consults me to contact my doctor.

I order a doctor’s appointment the second she leaves.

Anfald - The Attacks

Arthur får flere og flere rystelser hver dag.
Det ligner anfald. Jeg ved præcis hvornår det starter... En helt forfærdelig hakkende gråd kommer fra Arthur. Lyden borer sig gennem mig og jeg får kuldegysninger. Arthurs krop bliver pludselig helt stiv. Han knytter sine hænder og samler armene i opret position. Arthurs øjne, er helt åbne, de fyldes med angst... Jeg er sikker på, at anfaldet er meget ubehageligt for ham, jeg er sikker på, at han ikke forstår hvad der foregår, jeg tror måske det gør ondt på ham. De varer maksimalt et halvt minut. Han kan ikke selv stoppe anfaldet, men pludselig giver anfaldet slip på ham, hans arme falder ud til siderne og han græder som pisket. Jeg skynder mig altid at tage ham op, så jeg kan trøste ham, men gråden fortsætter.

The Attacks

Arthur has more and more tremors for every day that passes. They look like attacks. And I know exactly how they begin. Arthur starts with a horrible chopping cry. The sound drills through my body and gives me the chills. His body turns stiff and his arm cramps together infront of his him. His eyes are completely open and filled with (angst!) I am certain that the attacks are very uncomfortable for him. I am certain that he does not understand what is happening and I think that he attacks they might hurt him.

The attacks lasts no longer than 30 seconds. He can’t stop or control the attacks  himself – they just suddenly wear of. His arms settle down along his side and he starts to cry his eyes out. I always hurry to pick him up so I can comfort him – but he continues to cry.
